A probe in the accident of Indian Navy ship INS Sindhuratna says it was caused due to human error, reports CNN-IBN.

The inquiry report mentions deviation from standard operating procedures, sources said.

Last week, navy chief Admiral Devender Kumar Joshi resigned owning "moral responsibility" for warship and submarine accidents over the last six months, after the latest mishap on board submarine INS Sindhuratna off Mumbai.
